1) Trade Invoice
2) Account Opening Form
3) Terms of Agreement
Once the Account Opening form and Terms of Agreement have been completed, please forward a copy

When completing your payment please ensure to double check that the beneficiary banking details are
completed exactly as the enclosed banking instructions so as to avoid any delays.
For us to better track your payment, please write your invoice number (top left of the invoice page) in
the reference section of the transfer form.
Lastly please ensure that all banking fees are paid at both the sending and receiving end by the
sender. If a lesser amount is received the total share purchase amount will be adjusted accordingly.
